Identifikasi Drug Related Problems (DRPs) Pada Pasien Skizofrenia yang Mendapatkan Terapi Obat Psikotropika di Rumah Sakit Khusus Jiwa Soeprapto Provinsi Bengkulu

Abstract

Drug Related Problems (DRPs) are undesirable events involving drug therapy, either actual or potential, that may affect a patient's therapeutic outcomes and goals. Schizophrenia patients receiving long-term psychotropic drug therapy are at high risk of experiencing DRPs due to the complexity of their treatment regimens. This study aimed to identify the incidence and categories of Drug Related Problems (DRPs) among hospitalized Schizophrenia patients at Soeprapto Mental Hospital, Bengkulu Province, based on the PCNE V9.01 classification. This was a descriptive observational study using retrospective data from the medical records of Paranoid and Residual Schizophrenia patients during January–December 2025. Samples were selected using purposive sampling from a population of 1238, yielding 302 patients who met the inclusion criteria. DRPs were identified across the domains of drug selection, dosage form, and dose selection, referring to the British National Formulary (BNF) 2023. Results showed that DRPs occurred in 17 of 302 patients (5.6%), consisting of inappropriate drug combination in 12 patients (3.97%), underdosing in 3 patients (0.99%), and inappropriate drug selection according to guidelines in 2 patients (0.67%), while no DRPs were found in the dosage form category (100% appropriate). The risperidone-clozapine combination was the most frequent cause of inappropriate drug combination DRPs, posing risks of QTc prolongation and extrapyramidal effects.
