Electronic Medical Record (EMR) usability is an important determinant of the effectiveness of hospital information systems because health professionals interact with the interface repeatedly during clinical and administrative workflows. This study aimed to evaluate the conformity of an EMR user interface at Hospital X with the 17 user-interface design principles of Deborah J. Mayhew and to measure its usability using the System Usability Scale (SUS). A descriptive evaluative approach was used through direct observation of the EMR interface, checklist assessment, interview, and a SUS questionnaire administered to 10 active users. The observed variables included patient identity, newborn identity, payment method, and general consent. The findings showed that most of the 17 principles were fulfilled, but limitations remained in product compatibility, control, consistency, and responsiveness. Of 34 patient-identity variables, 18 were available and conformed to the reference while 16 were unavailable. For newborn identity, 5 of 6 variables were available; payment-method information was fully available; and 10 of 14 general-consent variables were available. The mean SUS score was 48.25, categorized as Not Acceptable, with a poor adjective rating and grade F. The findings indicate that interface improvement should prioritize regulatory data completeness, user control for data correction, consistent terminology, and clearer system feedback during loading. Periodic usability evaluation is recommended to maintain the relevance of the EMR interface to hospital workflows.
