Abstract

Collision accidents remain one of the major threats to maritime transportation safety, particularly in narrow waterways with dense vessel traffic. This study aims to analyze the factors causing the collision between SPB. Lebam and TB. Masada 25 in the Barito River and to identify preventive measures to reduce the risk of similar incidents. The research employed a qualitative descriptive approach using observation, interviews, and documentation as data collection techniques. Data were obtained during sea practice aboard SPB. Lebam and were analyzed through data reduction, data presentation, and conclusion drawing. The findings revealed that the collision was caused by several interrelated factors, including human error, ineffective navigational communication, inadequate implementation of proper lookout, failure to maintain safe speed, and the absence of pilot assistance in a compulsory pilotage area. In addition, the narrow channel characteristics and high traffic density of the Barito River contributed significantly to the complexity of navigation and increased collision risk. The study concludes that improving compliance with COLREG 1972, enhancing navigational communication, optimizing proper lookout practices, implementing Bridge Resource Management (BRM), and utilizing pilot services are essential measures to improve navigation safety and prevent future collision accidents in narrow waterways.
